That is because the NTLM patch has been fully integrated into the
current development sources of Squid.

As of yet, the latest NTLM developments has not been backported to
Squid-2.4 (or 2.3).

The current Squid development sources can be found at
http://www.squid-cache.org/Versions/v2/2.5/ or by using CVS as described
in http://www.squid-cache.org/Devel/cvs.html
