[SQU] ntlm

I still cannot get names in my access.log, I know squid is using
fakeauth_auth because the file is in use while squid is running, and the
logs says it spawns 5 children e.t.c.

Attached is my squid.conf, can anybody see why this is not working? Using
squid 5, with a NT PDC.
