The ntlm scheme is only available in Squid-2.5 or later. The
authentication in Squid-2.5 has undergone a major rewrite to support
other authentication schemes than Basic. Compared to this the other
changes in Squid-2.5 is quite minor...

Porting the SmartFilter patch to Squid-2.5 should be quite trivial.
